It looks to me like footswitches 3 or 4 can control MIDI hold, but there is no way to simply enable/disable MIDI transmission separate from hold with either of these switches. Am I right?

Map the chosen footswitch to an internal pedal, assigned to MIDI controller 007, on the global control channel of your external MIDI gear.
Set a min and max volume for the controller of "0" and what ever volume you want when the external gear is active.

I have had success with using a plain ol' M-Audio expression pedal set to "other" plugged straight into the controller jack. The VG sees the center position of the pot as MIDI off. It's somewhat counter-intuitive, because heel is on, center is off, and toe is hold. But it works very smoothly and predictably.

BTW, it only works set as controller 4.
