VG-8 - Background noise through amp(grounding issue)

VG-8, GK-2A

I'm having what appears to be a grounding issue with my VG8/GK2A.  There is a large amount of feedback and a muddy sound, particularly through an amp or (through the mix-out mono).

I've double checked and verified all the system settings, and manual and everything looks to be correct, so that doesn't appear to be the issue. 

The other issue I am having is the guitar only and mix channel is not coming through when selected.  I replaced the GK2 to guitar wire, and the guitar pickups will only come with some significant jiggling.

I'm hoping both of these issues is just with the GK2A, (or something I've missed in the setup) but I'm worried it could be with the VG.  Has anyone experienced this or have any suggestions before I start buying random parts 🤣?

Thanks man!  This could be it as it's been a while since I've used it, let alone not just  through MIDI.

I'm also looking into the driver settings as it's on a new guitar than it was previously, (in hind sight I that was pretty dumb of me not to do earlier 🤣)

factory settings:

1) Hold down F1, F3 and F5 while powering up.
2) Press F4 [Patches] (to restore only factory patches), F2 [System] (to
restore only factory system parameters,) or F6 [All] (to restore all factory
settings).

FWIW - there is no MIDI used on the GK-2A or the GK 13 pin cable and VG-8 does not function using MIDI Note on/off messages
